The 11 leave request form questions in this template.
In the order employees fill them in. The question about hours appears only for part-day requests.
The employee
-
Full name
Why ask it. Who is asking. Every request in the report carries a name.
-
Work email
Why ask it. Where the manager sends the decision, in a field made for e-mail addresses.
-
Team or department
Why ask it. Shows which team will be short. Turn it into a dropdown with your teams if you want the report split by team.
The leave
-
What type of leave are you requesting?
Why ask it. Five common types and an Other field. The form tells employees they don’t need to give a medical reason or other private details.
-
First day of leave
Why ask it. A date field, so every request arrives in the same format.
-
Last day of leave
Why ask it. The end of the leave. For a single day, the employee picks the same date twice.
-
Number of working days
Why ask it. The employee counts only the days they would normally work, which is the number you take off the leave balance.
-
Will you be away for full days or part of a day?
Why ask it. Part of a day opens the question about hours. Full days skip it.
-
Which hours will you be away?
Why ask it. Seen only in part-day requests, so the team knows when to expect the employee back.
Cover and handover
-
Who will cover your work, and what needs to be handed over?
Why ask it. The colleague who covers, urgent tasks, deadlines and where the files are. The handover is settled before the leave starts.
-
Anything else your manager should know?
Why ask it. Optional room for the rest, such as whether the employee can be reached in an emergency.

Who is away when
Export the requests to Excel or CSV on any plan and sort them by first day. Overlapping dates in one team show up before they become a problem.
A live leave tracker
On Basic and higher plans, send every request to Google Sheets as it arrives, and add a column for the decision.
Which leave types come up
The chart for the leave type question shows the mix of vacation, personal days and other leave. On Basic and higher plans, filter the requests by type or by team.
How to handle leave requests smoothly.
-
Say how much notice you need
Put your notice period on the welcome screen. Leave rules and notice periods differ by country and company, so write your own.
-
Say when they will hear back
Change the thank-you screen to say who decides and how soon, so nobody books a trip before the answer.
-
Confirm receipt automatically
On Basic and higher plans, an auto-reply e-mail tells the employee their request arrived.
-
Keep private details out
Leave the medical reason out of the form. The type of leave and the dates are enough to plan the team’s work.

Questions about this template
What should a leave request form include?
The employee’s name, work e-mail and team, the type of leave, the first and last day, the number of working days, the hours for part-day leave and who covers the work. This template asks all of it in eleven questions.
Who receives the requests?
Every request is saved in your account. Turn on e-mail notifications so you and your team get an e-mail with each new request. The number of recipients grows with the plan.
Can managers approve requests in the form?
No. The form collects requests, and the decision happens outside it. The manager replies to the employee’s work e-mail and records the leave in your HR system or in the exported spreadsheet.
Does the form count leave days or check the balance?
No. The employee enters the number of working days, and you check it against your policy. Leave entitlements and notice periods differ by country, so set your own rules in the welcome text.
Why doesn’t the form ask for a medical reason?
A manager needs the dates and the cover to plan the work, not health details. The leave type question tells employees they don’t need to give a medical reason or other private details.
Collect every leave request in one place.
Copy the form, name your leave types and put the link on the intranet.
