Contact details

  1. Full name

    Open answer Required

Why ask it. Whose order it is. It goes on the package or the pickup list.

  2. Email

    E-mail Required

Why ask it. Where you send the confirmation with the total and payment details, in a field made for e-mail addresses.

  3. Phone

    Phone number Required

Why ask it. For a quick call when an item is out of stock or the courier can’t find the door. The form tells customers why you ask.

The order

  1. How many of each would you like?

    Matrix Required

    • [Product 1]
    • [Product 2]
    • [Product 3]
    • [Product 4]
    • [Product 5]

    Scale: 0, 1, 2, 3, 4, 5+

Why ask it. Five products and their quantities on one screen. Put your product names in the rows; for more than five, the customer picks 5+ and writes the number in the notes.

  2. Size or variant

    Dropdown

    • [Variant 1]
    • [Variant 2]
    • [Variant 3]

Why ask it. The size, color or version the customer wants. Put your variants in the list, or delete it if every product comes in one version.

Delivery or pickup

  1. How would you like to receive your order?

    Single choice Required

    • Delivery
    • Pickup

Why ask it. Tells you whether to book a courier or set the order aside. Choosing Pickup skips the address.

  2. Delivery address

    Open answer Required

Why ask it. One open answer holds the street, apartment, city and postal code. The hint under the question lists what to include.

  3. Preferred delivery or pickup date

    Date Required

Why ask it. The day the customer wants the order. You confirm it when you write or call back.

Payment, invoice and notes

  1. How would you like to pay?

    Single choice Required

    • Cash on delivery or at pickup
    • Card on delivery or at pickup
    • Bank transfer (we'll send you the details)

Why ask it. Tells you whether to send bank details or have a card reader ready. The form takes no payment and says so under the question.

  2. Do you need an invoice issued to a company?

    Yes / No Required

    • Yes
    • No

Why ask it. Business customers say up front that they need an invoice. Choosing No skips the next field.

  3. Company name and tax ID

    Open answer Required

Why ask it. The details for the invoice, with the billing address. Only customers who answered Yes see it.

  4. Notes or special requests

How to work with incoming orders.

What to prepare

The matrix chart shows how many customers chose each quantity of each product. Plan stock and baking or packing by those numbers.

Deliveries and pickups apart

On Basic and higher plans, filter the orders by “How would you like to receive your order?” to get the courier’s list and the pickup list for the day.

Every order in a spreadsheet

Export the orders to Excel or CSV on any plan. On Basic and higher plans, each new order can also go straight to Google Sheets.

How to get complete orders.

  1. Say when you will confirm

Add one line to the welcome screen: when you reply with the total and how. Customers wait more calmly when they know.

  2. Name products the way you sell them

Use the names from your menu or catalog in the matrix rows, with the price in brackets if it helps the customer choose.

  3. Confirm the order arrived

On Basic and higher plans, an automatic reply tells the customer the order is in, right after they send it.

  4. Close it when you sell out

Close the form by hand on any plan. On Standard and higher plans it closes by itself on a date or after a set number of orders.

Questions about this template

Can customers pay in the form?

No. The form takes the order and has no card fields. The customer chooses cash or card on delivery or at pickup, or a bank transfer; you confirm the order and send the total and payment details by e-mail or phone.

How do I know when a new order arrives?

Turn on e-mail notifications in the form settings, on any plan. On Basic and higher plans you can also send each order to Google Sheets and an automatic reply to the customer.

Can I list more than five products?

Yes. Add rows to the matrix, or split a long catalog into several matrices by category. Customers who want more than five of an item pick 5+ and write the number in the notes.

Does the order form work on the Free plan?

Every field works on Free except the quantity matrix, which needs Basic or higher and is included in the 14-day Premium trial every new account starts with. On Free, replace the matrix with a dropdown of quantities for each product.
