Contact details
-
Full name
Why ask it. Who the appointment is for, and the name that goes in your calendar.
-
Email
Why ask it. Where the confirmation goes when the client prefers e-mail, in a field made for e-mail addresses.
-
Phone
Why ask it. For a quick call or message if something changes. The note under the question tells clients why you ask.
The visit
-
Which service would you like to book?
Why ask it. Replace the four placeholders with your services. Clients can pick more than one, so you know how long to block, and Other covers what is not on the list.
-
Have you been to us before?
Why ask it. A first visit may need more time or a short welcome. Regulars you already know.
Date and time
-
What date works best for you?
Why ask it. The day the client wants. Check it against your calendar before you reply.
-
What time of day do you prefer?
Why ask it. Narrows the day down without making the client guess which slots are free.
-
Another date that works for you
Why ask it. Optional. A backup in case the first day is taken, so you can confirm without another round of messages.
Confirmation and notes
-
How should we confirm your appointment?
Why ask it. You confirm the time through the channel the client actually reads.
-
Anything we should know before your appointment?
Why ask it. What exactly the client needs or a question for you. Read it before you confirm.

How to work with appointment requests.
When clients want to come
The chart for “What time of day do you prefer?” shows how requests split between morning, afternoon and evening. Plan shifts by those numbers.
Which services are booked
The chart for question four counts how often each service is requested. On Basic and higher plans, filter by first visit to see the new clients.
Every request in a spreadsheet
Export the requests to Excel or CSV on any plan. On Basic and higher plans, each new request can also go straight to Google Sheets.
How to get more appointment requests.
-
Say when you will confirm
The thank-you screen says you will be in touch soon. Replace “soon” with the time you can keep, such as within two hours on working days.
-
Name services as on your price list
Clients choose faster when they see the names and durations they know, such as “Haircut and styling, 60 min”.
-
Answer fast
Turn on e-mail notifications, on any plan, and confirm while the client is still choosing where to go. On Basic and higher plans, an automatic reply tells them the request is in.
-
Close it when you are away
Close the form by hand on any plan during holidays. On Standard and higher plans it closes by itself on a date or after a set number of requests.

Questions about this template
What should an appointment request form ask?
A name, an e-mail and a phone number, the service, whether the client has been to you before, a preferred date and time of day, a backup date and how to confirm. This template asks all of it in ten fields, eight of them required.
Can clients pick an exact time in the form?
Yes, with a Booking question on Basic and higher plans. You set your weekly hours or separate slots, the client sees only your free times in their own time zone, and the booking is created when they send the form. Every new account starts with a 14-day Premium trial that includes it.
How do I confirm the appointment?
You write or call the client through the channel they chose in question nine. The form collects the request and does not send texts or WhatsApp messages itself. On Basic and higher plans, an automatic e-mail tells the client their request arrived.
Can I add a Book button to my website?
Yes. Embed the form as a button, a pop-up, a side widget or a frame on your page, or share it as a link or a QR code. All of it works on every plan.
Where do new requests arrive?
Every request is saved in the form’s answers in your account. Turn on e-mail notifications to get each new one in your inbox: one recipient on Free, up to 3 on Basic, 5 on Standard and 10 on Premium.
