Free survey template

Training feedback form template. What people will use at work, and what to change before the next session.

Send it after a workshop, a corporate training or a webinar. Participants rate how useful the content was for their job, the level, the trainer and the length, say how confident they feel about applying it and name the first thing they will use. An NPS question with its own follow-up for each group closes the survey. Anonymous mode is already on in this template.

The 11 training feedback questions in this template.

In the order participants see them. The survey asks for no name, and you can change any question.

The training

  1. How did you attend the training?

    Single choice Required

    • In person
    • Online, live
    • Watched the recording

Why ask it. In-person, live online and recorded sessions can get very different answers. This one lets you compare the formats instead of mixing them into one average.

  2. Overall, how satisfied are you with the training?

    Single choice Required

    • Very dissatisfied
    • Dissatisfied
    • Neutral
    • Satisfied
    • Very satisfied

Why ask it. One overall score per session, easy to track from one training to the next.

  3. How useful was the content for your job?

    Single choice Required

    • Not at all useful
    • Slightly useful
    • Moderately useful
    • Very useful
    • Extremely useful

Why ask it. Enjoying a session and using it at work are different things. A training people liked but cannot use shows up here.

  4. How was the level of the material for you?

    Single choice Required

    • Too basic
    • A bit basic
    • Just right
    • A bit advanced
    • Too advanced

Why ask it. Five steps from too basic to too advanced. When answers split at both ends, the group needs two levels, not a new trainer.

The trainer and the format

  1. How would you rate the trainer?

    Rating 1–5 Required

Why ask it. A quick star rating for the person who ran the session. Read it next to the next question to see what the trainer did well.

  2. What helped you learn the most?

    Multiple choice

    • The trainer's explanations
    • Real-life examples
    • Exercises and practice
    • Discussion and Q&A
    • Materials and slides
    • Other, in their own words

Why ask it. Shows which parts of the session to keep. Optional, with room for an answer of their own.

  3. How was the length of the training?

    Single choice Required

    • Too short
    • About right
    • Too long

Why ask it. Three answers, enough to decide whether the next session gets shorter, longer or stays as it is.

Back at work

  1. How confident are you that you can apply what you learned?

    Single choice Required

    • Not at all confident
    • Slightly confident
    • Moderately confident
    • Very confident
    • Completely confident

Why ask it. Shows whether the training will change how people work. Low answers mean participants need more practice or a follow-up session.

  2. What is one thing you'll apply in your work first?

    Open answer

Why ask it. Asking for one concrete thing makes people connect the session to their job, and gives you real examples to show managers.

  3. What was missing, or what would you change?

    Open answer

Why ask it. The to-do list for the next version of the training. Optional, so people write only when they have something to say.

Recommendation

  1. How likely are you to recommend this training to a colleague?

NPS scale 0–10 Required 0 – Not at all likely, 10 – Extremely likely

Then one open question, depending on the score:

    • 0–6 What didn't work for you?
    • 7–8 What would make it a 10?
    • 9–10 What made it worth your time?

Why ask it. The standard Net Promoter Score question, asked about the training. The follow-up appears under the scale once a score is picked, worded for the participant’s group, so every score comes with its reason.

How to collect training feedback with this template.

  1. 1

    Copy the form

Click “Use this template” and create a free account or log in. The form is copied to your account with all 11 questions, the three NPS follow-ups and anonymous mode turned on. The copy waits in your survey list as a draft.

  2. 2

    Put in the training name

Replace [Training] on the welcome screen with the name of the session. The welcome screen already tells participants the survey is anonymous. For a session with no recording, delete the last option of the first question.

  3. 3

Share it at the end of the session

Show the QR code on the last slide, paste the link into the webinar chat, or e-mail it to participants the same day.

  4. 4

    Read the results

Every closed question gets a chart, and the report shows the Net Promoter Score with the share of detractors, passives and promoters. Share a read-only report link with the trainer or export the data.

How to read training feedback results.

Useful or just pleasant

Compare overall satisfaction with usefulness for the job and confidence to apply it. High satisfaction with low usefulness means the session needs more practice and fewer slides.

Level, length and format

Answers that pile up at one end of the level or length question tell you what to change for the next group. On Basic and higher plans, filter by the first question to compare in-person, live online and recorded sessions.

What people will apply

The answers about what people will use first show what the training changed. What was missing becomes the plan for the next version. Export everything to Excel, CSV or SPSS on any plan.

How to get training feedback you can use.

  1. Ask before people leave

Give five minutes at the end of the session and show the QR code. Fewer people answer once they are back at their desks.

  2. Keep it anonymous, and say so

Anonymous mode is already on in this template. Keep it on, and tell participants at the start of the session that the survey is anonymous.

  3. Check again a month later

Four to six weeks after the training, send a short follow-up about what people actually used. Compare it with what they planned to apply.

  4. Show what changed

Open the next session with one thing you changed because of the feedback. People answer more carefully when they see it matters.

  • Would you prefer this training in person or online?

A few weeks later

  • Which part of the training have you used at work?
  • What stopped you from applying the rest?
  • What follow-up would help you most?

Questions about this template

What questions should a post-training survey ask?

Ask how useful the content was for the job, whether the level and length were right, how the trainer did, what helped people learn and how confident they feel about applying it. Then ask what they will use first and what was missing. This template asks all of it in 11 questions and ends with NPS.

Is the training feedback form anonymous?

Yes. The form asks for no name, and anonymous mode is already on in this template. It works on every plan, including Free.

Does it work for webinars and online training?

Yes. The first question asks how people attended, so you can compare in-person, live online and recorded sessions. Paste the link into the webinar chat or the follow-up e-mail.

How is it different from a course evaluation form?

This form is for employees after a workshop, a corporate training or a webinar, and asks how useful it is for their job. For a school or university course, use the course evaluation form template.

Is the training feedback form free?

Yes. Every question, the NPS question with its follow-ups and anonymous mode work on the Free plan, and the report calculates the score on every plan.
