Contact details

  1. Full name

    Open answer Required

Why ask it. Who the customer is, and the name you greet them with on the next visit.

  2. Email

    E-mail Required

Why ask it. Where your messages and offers go, in a field made for e-mail addresses.

  3. Phone

    Phone number Required

Why ask it. For updates about visits and orders. The note under the question asks for the WhatsApp number if the customer uses it.

About the customer

  1. City or neighborhood

    Open answer

Why ask it. Shows which areas your customers come from, for local ads and delivery zones. Optional, so nobody stops here.

  2. How did you hear about us?

    Single choice

    • A friend or family member
    • Instagram or Facebook
    • TikTok
    • Google or Google Maps
    • I passed by your location
    • Other, in their own words

Why ask it. Tells you which channel brings new customers: word of mouth, social media, search or the shop window. Other catches the rest.

  3. What are you interested in?

    Multiple choice

    • [Product or service 1]
    • [Product or service 2]
    • [Product or service 3]
    • [Product or service 4]
    • [Product or service 5]

Why ask it. Replace the five placeholders with what you sell. Customers pick everything that interests them, so each offer goes to the people who asked for it.

Contact and offers

  1. How would you like to hear from us?

    Multiple choice Required

    • Email
    • Text or WhatsApp message
    • Phone call
    • I'd rather not be contacted

Why ask it. Write where the customer reads, and leave alone the people who chose the last option.

  2. Would you like to receive our offers and news?

    Yes / No Required

    • Yes
    • No

Why ask it. A plain question about consent to offers, with a note that they can unsubscribe at any time. A No skips the birthday question.

  3. When is your birthday?

    Date

Why ask it. Optional and only for offers: the note says a birthday offer will follow. Only customers who said Yes to offers see it.

  4. Anything else we should know?

    Open answer

Why ask it. Preferences or the best time to call, in the customer’s own words. Read it before the next visit.

How to work with customer registrations.

Where new customers come from

The chart for “How did you hear about us?” counts each channel. Spend your next promotion where the most customers found you.

Offers for the right people

The chart for question six shows what customers want most. On Basic and higher plans, filter by a product and by Yes to offers to get the list for that offer.

Your customer list in a spreadsheet

Export the registrations to Excel or CSV on any plan and sort birthdays by month. On Basic and higher plans, each new customer can also go straight to Google Sheets.

How to get more customers to register.

  1. Ask while they wait

Put the QR code where customers sit or stand in line. Two minutes of waiting is enough to fill the form on a phone.

  2. Say what they get

The welcome screen promises faster service and offers that suit them. Add the perk you really give, such as a discount on the next visit.

  3. Name your real products

Use the names from your price list or menu in question six. Delete the placeholders you don’t need.

  4. Welcome them right away

On Basic and higher plans, an automatic reply thanks each customer by e-mail the moment they register.

Questions about this template

What should a customer registration form ask?

A name, an e-mail and a phone number, where the customer lives, how they found you, what interests them, how they want to hear from you and whether they want your offers. This template asks all of it in ten fields, five of them required.

How do customers fill it in at the shop?

Print the QR code for the counter, and customers fill in the form on their own phones. You can also open the link on a tablet at the reception desk. Every registration lands in your account.

How do I know who agreed to receive offers?

Question eight asks it directly, with a note that they can unsubscribe at any time. On Basic and higher plans, filter the registrations by Yes. On any plan, export them to Excel or CSV and load the list into the tool you send offers with.

Do customers have to give their birthday?

No. The question is optional, says it is only for a birthday offer, and customers who said No to offers never see it.

Where do new registrations arrive?

Every registration is saved in the form’s answers in your account. Turn on e-mail notifications to get each new one in your inbox: one recipient on Free, up to 3 on Basic, 5 on Standard and 10 on Premium.
