The template’s real survey on a phone.
- 7 ready questionsAbout 2 min on a phone.
- Yours to changeQuestions, answers and logic.
- A ready reportA chart for every closed question.
- Export on every planCSV, Excel and SPSS.
Unhappy customers often leave without a word. A two-minute form gives them a place to say what went wrong, and you an e-mail as soon as they do.
The 7 feedback form questions in this template.
In the order people meet them. One logic rule skips the e-mail for those who don’t want a reply, and you can change any question.
The experience
-
What is your feedback about?
Why ask it. Sorts every entry from the first click, so feedback about the website doesn’t get mixed with feedback about your staff. Swap the five options for what you actually offer.
-
Overall, how would you rate your experience?
Why ask it. One star score per entry. Its average is the number to watch from month to month.
-
How did it compare with what you expected?
Why ask it. Four stars from someone who expected more is a warning. This question shows whether your promise and your delivery match.
In their own words
-
What did you like most?
Why ask it. What to keep when you change things, and what to say in your marketing, in your customers’ own words.
-
What should we improve?
Why ask it. The fix list. The hint under the question says that even small details help, so people write the things they would never call about.
Follow-up
-
May we contact you about your feedback?
Why ask it. Asks before it asks for an address. People who answer No send the form right away.
-
Your email
Why ask it. Where you reply to a complaint or say thanks for an idea. Only people who agreed to be contacted see it.

- 4
Read and reply
Turn on e-mail notifications, so you and your team get an e-mail with every entry. Reply to the people who left an address, and open the report for the charts.
How to work with the feedback you get.
Your average rating
The report shows how many people gave each number of stars. Track the average and the share of 4 and 5 stars from month to month.
Where the low scores come from
On Basic and higher plans, filter the entries by what the feedback is about or by “Worse than I expected” to see which part of your business collects the complaints.
What people ask you to fix
Read the answers to “What should we improve?” side by side. Export everything to Excel or CSV on any plan and count the requests that repeat.
How to get more feedback.
-
Ask while it is fresh
Send the link the same day as the purchase, the visit or the class. A week later people remember less and answer less often.
-
Say it takes two minutes
The welcome screen says 1–2 minutes. Keep that line: people start a form when they know it is short.
-
Reply to every complaint
A personal answer to a low score shows the customer someone read it. E-mail notifications for each entry work on every plan.
-
Keep it in one place
Use one form on the website, the receipt and the QR code, so all feedback lands in one report.

Questions about this template
What should a feedback form ask?
What the feedback is about, one overall rating, how the experience compared with expectations, what the person liked and what to improve, and whether you may reply. This template asks all of it in seven questions and takes about two minutes.
How is it different from a customer satisfaction survey?
The feedback form is short and open: anyone can send it at any time, about anything. The customer satisfaction survey template asks fourteen questions about six parts of your service and gives you a CSAT score to track by quarter.
How do I know when new feedback arrives?
Turn on e-mail notifications in the form settings, on any plan: you and your team get an e-mail with each entry. On Basic and higher plans, entries can also go to Google Sheets, a webhook or Zapier.
Can people leave feedback anonymously?
Yes. The e-mail question appears only to people who agree to be contacted. For fully anonymous feedback, delete the last two questions and turn on anonymous mode, which works on every plan.
Does the feedback form work on the Free plan?
Yes. Every question in it, the star rating included, works on Free, together with the logic rule, e-mail notifications, embedding on your site and export to Excel or CSV.
