The 11 event registration form questions in this template.

In the order attendees meet them. Put your own sessions in, delete what your event doesn’t need and add your own.

About the attendee

  1. Full name

    Open answer Required

Why ask it. Who is coming. It goes on the attendee list and the name badge.

  2. Email

    E-mail Required

Why ask it. Where the confirmation and the event details go, in a field made for e-mail addresses. The form tells attendees so under the question.

  3. Phone

    Phone number

Why ask it. For a last-minute change of room or time. It is optional, so people who would rather not share a number still register.

  4. Organization

    Open answer

Why ask it. Shows which companies are in the room, for sponsors, seating and name badges. Delete it for a public or family event.

  5. Job title

    Open answer

Why ask it. Tells speakers who they are talking to and helps people find each other at networking. Delete it together with the organization if neither matters.

Sessions and access

  1. Which sessions will you attend?

    Multiple choice Required

    • Opening keynote
    • Morning sessions
    • Afternoon sessions
    • Workshop
    • Networking reception

Why ask it. A head count for every part of the program, so each room has enough seats and materials. Replace the five options with your own agenda.

  2. Do you need anything to take part comfortably?

    Open answer

Why ask it. Step-free access, captions or a sign language interpreter take time to arrange. Asking at registration gives you that time.

Format and catering

  1. How will you attend?

    Single choice Required

    • In person
    • Online

Why ask it. Splits the list into the venue head count and the people who get the stream link. Choosing Online skips the dietary question.

  2. Do you have any dietary requirements?

    Multiple choice

    • None
    • Vegetarian
    • Vegan
    • Gluten-free
    • Halal
    • Kosher
    • Other, in their own words

Why ask it. Exact numbers for the caterer. Allergies go in the Other field at the end of the list, as the hint under the question asks.

Promotion and follow-up

  1. How did you hear about this event?

    Single choice

    • Email
    • Social media
    • A colleague or friend
    • Our website
    • A partner organization
    • Other, in their own words

Why ask it. Shows which channel fills the seats. Put the next event’s promotion budget there.

  2. Would you like to hear about our future events?

    Yes / No

    • Yes
    • No

Why ask it. A list of people who asked to hear from you, ready for the next announcement.

How to work with event registrations.

A head count for every session

The chart for “Which sessions will you attend?” shows how many people chose each part of the program. Book rooms and print materials by those numbers.

Two lists in one form

On Basic and higher plans, filter registrations by “How will you attend?” to get the venue list for badges and the caterer, and the online list for the stream link.

A list for check-in

Export the registrations to Excel or CSV on any plan, with names, organizations and access needs side by side, and print the check-in sheet.

How to get more people to register.

  1. Fill in the placeholders first

The welcome screen says [Event name], [Date] and [Venue or online]. Replace all three before you publish, so guests know what they are signing up for.

  2. Confirm right away

The thank-you screen promises the event details by e-mail. On Basic and higher plans, an automatic reply sends them the moment someone registers.

  3. Close it when the seats run out

Close the form by hand on any plan. On Standard and higher plans it closes by itself on a date or after a set number of registrations.

  4. Keep it short on a phone

Most people register from a link on their phone. Two minutes is the budget, so delete any field you won’t use on the day.

  • Would you like a list of nearby hotels?
  • What time do you expect to arrive?

Before the event

  • What do you hope to take away from this event?
  • Which question would you like the speakers to answer?
  • May we include your name on the attendee list?

Questions about this template

What should an event registration form ask?

Name and e-mail, the sessions the person will attend, whether they come in person or online, anything they need to take part comfortably, dietary needs and how they heard about the event. This template asks all of it in eleven fields, four of them required.

Can attendees get a confirmation e-mail?

Yes. On Basic and higher plans, an automatic reply sends each person a confirmation with the event details right after they register. Every new account starts with a 14-day Premium trial that includes it.

Can registration close when the event is full?

Yes. Close the form by hand on any plan. On Standard and higher plans, set it to close after a number of registrations or on a date.

Does it work for hybrid events?

Yes. Question eight asks whether the person comes in person or online, and online guests skip the dietary question. On Basic and higher plans, filter the registrations to get each list.

How do I share it?

Put the link in your invitation e-mail and social posts, embed the form on the event page, or print the QR code on posters and flyers. Every registration lands in your account.
