The 9 gym feedback form questions in this template.
In the order members meet them. Swap the options for your own classes and ad channels, and add questions of your own.
How members found you
-
Where did you find out about our gym?
Why ask it. Shows which channel brings members in. Spend the next ad budget where the bar is tallest; the Other field catches what you didn’t list.
-
How often do you go to the gym?
Why ask it. Regulars and occasional visitors judge a gym differently. This is the question you filter by later.
Workouts and equipment
-
Which additional kinds of exercises do you prefer?
Why ask it. Which classes members want besides the weights floor. Replace the five options with the classes you run or plan to add.
-
Evaluate the gym equipment
Why ask it. A 1–5 star score for the machines and free weights. A low one tells you what to repair or replace first.
Price and staff
-
Are you satisfied with the pricing?
Why ask it. A plain yes or no on the membership price. If many say no, look for the reason in the last question.
-
Evaluate our staff
Why ask it. Trainers and front desk in one score. Split it into two questions if you want to rate them separately.
The overall verdict
-
Overall, to what extent are you satisfied with the gym?
Why ask it. One number for the whole gym, to compare from quarter to quarter. The share of 4s and 5s is your CSAT.
-
Would you recommend our gym to a friend?
Why ask it. A simple loyalty check. Track the share of yes answers next to the satisfaction score.
-
Share your ideas and suggestions on how we can improve the gym
Why ask it. Ideas in members’ own words: new classes, opening hours, the locker room. Read them before you plan the next purchase.

How to read gym feedback results.
Equipment or staff first
The two 1–5 star ratings sit side by side in the report, so you see at once whether the machines or the team need work.
Regulars against occasional visitors
On Basic and higher plans, filter the answers by visit frequency to see whether people who train three times a week rate the gym differently from those who come a few times a month.
Where new members come from
The chart for the first question shows which ads work. Export everything to Excel or CSV on any plan and compare it with your sign-ups.
How to get more members to answer.
-
Ask right after a workout
A QR code at the exit or by the lockers catches members while the session is fresh.
-
Keep it nameless
The template asks for no name or phone number, so members can say what they think of the staff.
-
Repeat it every quarter
Same nine questions, same moment. The satisfaction score shows whether your changes worked.
-
Show what changed
Put a note on the board: what members asked for and what you did. The next survey gets more answers.

Questions about this template
What questions should a gym feedback form ask?
How members found you, how often they train, which activities they like, how they rate the equipment and the staff, whether the price suits them, how satisfied they are overall, whether they would recommend you and what they would improve. This template asks all of it in nine questions.
Is this a fitness assessment form?
No. Here members assess the gym: the equipment, the staff and the price. A fitness assessment of a new member, with goals, health history and measurements, is a different form. You can add those questions to this one in the builder or start a separate form.
Does it work on the Free plan?
Yes. Every question type in this template works on Free, and so do logic, charts and export. Filtering the answers by visit frequency starts on Basic.
Do members have to give their name?
No. The template asks for no name, e-mail or phone number. If you want to call members back about their ideas, add an optional contact question at the end.
How do I share it with members?
Print the QR code for the front desk, the mirrors or the lockers, send the link in your newsletter or members’ chat, or embed the form on your website.
Ask your members what to fix next.
