Secret Santa form template. Everyone signs up, and every giver knows what their person would like.

Who’s joining, what each person enjoys, what they’d rather not get, allergies, diet, size, three gift ideas and where to ship a gift. Copy the form, put your budget and deadline on the welcome screen and share the link. The draw happens outside the form, with names in a hat or a random name picker. Then you send each giver only their person’s gift answers.

The 13 Secret Santa form questions in this template.

In the order participants meet them. Put your budget and deadline in place of [Budget] and [Date] on the welcome screen, and change any question.

Who’s joining

  1. Do you want to join the Secret Santa this year?

    Single choice Required

    • Yes, count me in
    • Not this year

Why ask it. Who’s in this year. “Not this year” ends the form right there, with no reason asked.

  2. Your name, as your Secret Santa knows you

    Open answer Required

Why ask it. The name that goes into the draw, the one colleagues actually use.

  3. Your email

    E-mail Required

Why ask it. Where you tell each person who they’re giving to. Their Secret Santa never sees it, and the question says so.

Gift preferences

  1. What do you enjoy in your free time?

    Multiple choice Required

    • Cooking
    • Reading
    • Gaming
    • Sports and fitness
    • Plants and gardening
    • Music
    • Crafts and DIY
    • Travel
    • Tech and gadgets
    • Other, in their own words

Why ask it. Hobbies point the giver to a gift. Other catches the ones the list misses.

  2. Which of these would you rather not get?

    Multiple choice

    • Scented candles or soaps
    • Alcohol
    • Sweets
    • Joke gifts
    • Decorative items
    • Clothing
    • None of these
    • Other, in their own words

Why ask it. Saves a gift from the back of a cupboard. “None of these” keeps it quick for people who like everything.

Allergies, diet and size

  1. Do you have any allergies or intolerances?

    Multiple choice

    • None
    • Peanuts
    • Tree nuts
    • Milk
    • Eggs
    • Wheat or gluten
    • Soy
    • Sesame
    • Fish or shellfish
    • Fragrances
    • Wool
    • Prefer not to say
    • Other, in their own words

Why ask it. Optional, with “Prefer not to say”. It goes to the giver with the gift answers, so nobody gets food or a scent they can’t use.

  2. Do you follow a diet your Secret Santa should know about?

    Multiple choice

    • No special diet
    • Vegetarian
    • Vegan
    • Halal
    • Kosher
    • Low sugar
    • Prefer not to say
    • Other, in their own words

Why ask it. Optional, with “Prefer not to say”. A food gift then fits what the person eats.

  3. Your size for tops and sweaters

    Single choice

    • XS
    • S
    • M
    • L
    • XL
    • XXL
    • Prefer not to say
    • Other, in their own words

Why ask it. Optional. It helps a giver thinking of a sweater or a T-shirt; people who skip it would rather not get clothes.

Gift ideas and delivery

  1. Three gift ideas within the budget

    Open answer

Why ask it. The most useful answer for the giver: three ideas within the budget, or a link to a wish list.

  2. Will you be at the exchange in person?

    Single choice Required

    • Yes, in person
    • No, my gift needs shipping
    • Not sure yet

Why ask it. Splits the gifts handed over in person from the parcels. “Yes, in person” and “Not sure yet” skip the shipping questions.

  3. Where should your gift be shipped?

    Open answer

Why ask it. Optional, and asked only of people whose gift needs shipping. It is used for that parcel and deleted after the exchange, as the question says.

  4. How should your gift reach you?

    Single choice

    • Through the organizer, so my Secret Santa doesn't see my address
    • Directly from my Secret Santa, who will see my address

Why ask it. The person decides who sees the address: it comes through you and the giver never sees it, or straight from the giver.

  5. Anything else your Secret Santa should know?

    Open answer

Why ask it. Room for what the list missed, like a favorite color or “no more mugs, please”.

How to run a Secret Santa with this form.

  1. 1

    Copy the form

Click “Use this template” and create a free account or log in. The form is copied to your account with all 13 questions and its skip rules. The copy waits in your survey list as a draft.

  2. 2

    Add your budget and deadline

Replace [Budget] and [Date] on the welcome screen. It already tells people who sees which answers and that answers are deleted after the exchange: keep that promise, or change the text to match what you will do.

  3. 3

    Share the link

Post it in the team chat or send it by e-mail, and print the QR code for the office kitchen. Turn on e-mail notifications to see each sign-up as it arrives.

  4. 4

Draw names and send the answers

Close the form on the deadline and export the answers. Draw names outside the form, in a hat or with a random name picker, and send each giver only their person’s gift answers: never the e-mail, and the address only if the person chose direct delivery.

How to use the Secret Santa answers.

The list for the draw

Export the answers to Excel or CSV on any plan: one row per person. Keep the rows of people who said “Yes, count me in”, and you have the names for the hat.

One card per giver

Copy each person’s hobbies, gifts to avoid, allergies, diet, size, gift ideas and notes into the message to their giver. Leave out the e-mail, and the address too, unless the person chose to get the gift directly from their Secret Santa.

Parcels to plan

The chart for “Will you be at the exchange in person?” shows how many gifts need shipping. People who answered “Not sure yet” may tell you later, so check with them before the exchange.

How to run a fair and friendly Secret Santa.

  1. Set one budget

Write the amount on the welcome screen in place of [Budget]. One number for everyone keeps the gifts even, and nobody overspends.

  2. Keep joining optional

“Not this year” is a full answer. Don’t ask why, and don’t list who stayed out.

  3. Give it a deadline

A week before the draw is enough. Send one reminder, then close the form by hand on any plan, or by date on Standard and higher plans.

  4. Delete the answers afterwards

The welcome screen promises it. Once the gifts are exchanged, delete the answers in your account, the exported file and any notification e-mails.

Questions about this template

What should a Secret Santa form ask?

Whether the person is joining, their name and e-mail, what they enjoy, what they’d rather not get, allergies and diet, their size, a few gift ideas within the budget and how the gift should reach them. This template asks all of it in thirteen questions, and eight of them are optional.

Does the form draw names?

No. The form collects sign-ups and gift answers, and the draw happens outside it, with names in a hat or a random name picker. After the draw, you e-mail each giver their person’s name and gift answers yourself.

Who sees the answers?

You, as the organizer, see every answer in your account. Each giver gets only what you send them: the welcome screen promises the gift answers without the e-mail, and the address only when the person chose direct delivery. Keep to that when you send the cards.

Can people join from other offices or cities?

Yes. Anyone who can’t make the exchange in person picks “No, my gift needs shipping” and gives an address; a pick-up point works too. The next question lets them decide whether the parcel comes through you or straight from their Secret Santa.

How do I share the form?

Send the link in the team chat or by e-mail, or print the QR code for the office. Every sign-up lands in your account, and e-mail notifications tell you when a new one arrives.
