First contact
-
What's your name?
Why ask it. Who signed up. It is required, so no registration arrives without a name.
-
How did you find out about the volunteer project of our organization?
Why ask it. Shows which recruiting channel brings people. Post your next call for volunteers there; the Other field catches the rest.
-
Do you have experience as a volunteer?
Why ask it. Separates first-timers, who need an introduction, from people who already know how volunteering works.
Skills and background
-
What kind of work would you like to do at our organization?
Why ask it. The heart of the form: which tasks each person will take on. Replace the seven options with the roles your project actually needs.
-
Tell us about your skills and hobbies
Why ask it. Turns up skills you did not think to list, from first aid to fixing bikes.
-
What's your educational background?
Why ask it. Helps match people to tasks that need training, such as tutoring. Delete it if no task depends on it.
-
What are you doing now?
Why ask it. Students, employees and retirees have different free time. It hints at who can come on a weekday morning.
Availability and contact details
-
What kind of schedule would you prefer as a volunteer?
Why ask it. The days and hours each person can give, in their own words. Read it before you plan shifts.
-
Your email:
Why ask it. Where the welcome message and the shift schedule go. Required, in a field made for e-mail addresses.
-
Your phone number:
Why ask it. For the reminder call the day before a shift, in a field made for phone numbers.
-
Links to your social media accounts:
Why ask it. Lets you see who is joining and tag volunteers when you post about the project. Delete it if you don’t need it.
About the volunteer
-
Your gender:
Why ask it. Useful when a task calls for it, such as support at a women’s shelter. Anyone can pick “Would rather not say”.
-
Please specify your age:
Why ask it. Shows the age mix of your team. The options start at 18; add one for younger volunteers if your project accepts them.

Questions about this template
What should a volunteer registration form ask?
Contact details, the kind of work the person wants to do, their skills and past volunteering, when they are free and how they heard about you. This template asks all of it in thirteen questions, four of them required.
Can I use it as a sign-up form for one event?
Yes. Delete the education and social media questions, turn the schedule question into a choice of your event’s shifts, and the form gets shorter. Any question deletes in one click.
How do I know when someone signs up?
Turn on e-mail notifications in the form settings, on any plan. On Basic and higher plans you can also send each registration to Google Sheets and send volunteers an automatic reply.
Can volunteers sign a waiver?
No. SurveyNinja has no e-signature. Link to your waiver in the form and collect signatures at orientation. On Standard and higher plans, a consent checkbox on the submit button records that the volunteer accepted your terms.
How do I share it with volunteers?
Post the link on social media, embed the form on your website, or print the QR code on flyers and posters. Every registration lands in your account.
