How they use it

  1. How often do you use [Product]?

    Single choice Required

    • Every day
    • A few times a week
    • A few times a month
    • Less than once a month
    • I no longer use it

Why ask it. Separates daily users from occasional ones, so you can read every other answer by usage. I no longer use it skips straight to the question about what is missing.

  2. What do you mainly use [Product] for?

    Single choice

    • Work
    • My own business
    • Personal or home use
    • Study
    • Other, in their own words

Why ask it. Shows who your users are. Replace these four options with your product’s own use cases before you send the survey, and keep Other for the ones you did not think of.

Satisfaction and ease of use

  1. Overall, how satisfied are you with [Product]?

    Single choice Required

    • Very dissatisfied
    • Dissatisfied
    • Neutral
    • Satisfied
    • Very satisfied

Why ask it. One satisfaction score to track from release to release. Compare it by how often people use the product.

  2. How easy is it to use [Product]?

    Single choice Required

    • Very difficult
    • Difficult
    • Neither easy nor difficult
    • Easy
    • Very easy

Why ask it. Ease of use in five words. A low score from daily users points to friction they work around every day.

What it is worth to them

  1. Which feature or part of [Product] is most useful to you?

    Open answer

Why ask it. What users name first, in their own words. That wording often makes better marketing copy than your own.

  2. How would you feel if you could no longer use [Product]?

    Single choice Required

    • Very disappointed
    • Somewhat disappointed
    • Not disappointed

Why ask it. The product-market fit question. The share of Very disappointed answers shows how much the product matters to its users. Former users skip it, so the share counts active users only.

  3. How likely are you to recommend [Product] to a friend or colleague?

NPS scale 0–10 Required 0 – Not at all likely, 10 – Extremely likely

Then one open question, depending on the score:

    • 0–6 What lets you down about [Product]?
    • 7–8 What would make [Product] a 10 for you?
    • 9–10 What would you tell a friend about [Product]?

Why ask it. The standard Net Promoter Score question. The follow-up appears under the scale once a score is picked, worded for the user’s group, so every score comes with its reason.

What to build next

  1. What is missing from [Product], or what should work better?

    Open answer

Why ask it. The open question for your roadmap. People who stopped using the product land here too, and the hint under the question asks them why they stopped.

Follow-up

  1. May we contact you to talk about your answers?

    Yes / No Required

    • Yes
    • No

Why ask it. Lets you follow up with users who have more to say. No ends the survey here.

  2. Your email

    E-mail Required

Why ask it. Asked only of users who said yes, so you have a way to reach them.

How to read product feedback results.

Product-market fit

Look at the share of Very disappointed answers to the sixth question and track it after each major release. On Basic and higher plans, filter it by main use to see which users the product fits best.

NPS and its reasons

NPS is the share of promoters (9–10) minus the share of detractors (0–6), from −100 to 100. Promoters tell you what to say about the product, detractors tell you what to fix.

What to build next

Group the answers to the last open question into themes and count them. Export everything to Excel, CSV or SPSS on any plan to tag the answers.

How to get product feedback you can use.

  1. Ask people who have used it

Send the survey to users who have had the product for a few weeks, so they answer from experience.

  2. Write your own use cases

The second question only helps when the options are yours. Replace them before you send the survey.

  3. Ask former users too

Include people who stopped using the product. The survey sends them straight to the question about what is missing.

  4. Close the loop

Tell users what you changed because of their answers. It gives them a reason to answer the next survey.

Questions about this template

What questions should a product feedback survey ask?

How often and what for people use the product, how satisfied they are, how easy it is, which part they value, how they would feel without it, whether they would recommend it and what is missing. This template asks all of it in ten questions.

Does it work for a physical product?

Yes. The questions talk about [Product], not about software. Put in your product’s name, replace the use cases in the second question and print the QR code on the packaging or the receipt.

What is the product-market fit question?

It asks how users would feel if they could no longer use the product: very, somewhat or not disappointed. The more users answer Very disappointed, the more the product matters to them. In this template only active users see it.

How is it different from a customer satisfaction survey?

A customer satisfaction survey asks about the company: service, price, delivery. This one asks about the product itself: how it is used, what is valuable and what is missing.

Can I use it on the Free plan?

Yes. Every question in this template, the NPS question with its follow-ups included, works on the Free plan, and the report calculates the NPS on every plan.
